In December 2022, there were 13 fatal workplace accidents in Shanghai"s industrial, mining, commercial and trading sectors with 13 deaths.
Some Accidents in December
On December 1, in a shop of Lane 468, Jianguo Road W., an operator from Sundart Engineering & Contracting (Shanghai) Limited fell off a scaffold tower and died.
On December 2, at the 4th floor, central plaza of Global Harbor shopping mall at No.3300 Zhongshan Road N., a site manager of Jiangsu Guanya Construction Engineering Co. Ltd. was walking through a glass plant road under repairment. The manager did not notice a piece of glass had been removed, fell through the hole and died.
On December 2, at a general production workshop of Shanghai Tongcheng Auto Parts Co. Ltd. at No.44 Honglan Road, an operator was hit by a robotic arm and died.
On December 3, an operator from Suzhou Changbang Material Recycle Co. Ltd. was moving an equipment at No.3966 Jindu Road, fell off the fire escape, and died.
On December 13, at the paint workshop of Changxing Branch of Shanghai Zhenhua Heavy Industries Co. Ltd. at No.666 Fengbin Road, an operator was directing a forklift moving materials. The materials fell off and hit the operator, who died on spot.
Workplace accidents in previous Decembers
“2021·12·15” Machinery Injury Accident at No.9 Building, No.625, Rongchang Road
Summary of the accident:
At around 17:00, December 15, 2021, at a workshop of Shanghai Liandong Technology Co. Ltd. at No.9 Building, No.625, Rongchang Road, Jinshan District, the head manager of the factory, who was also in charge of its work safety and technology, inserted a self-made steel pipe into a drafter machine for extruding. The pipe in his hand got stuck in the rollers, which caused the manager to lose balance. His head hit the steel rollers, and he died from the head injury.
Direct cause:
Mr. Zhu, who was the head manager of the factory as well as in charge of its work safety and technology, lacked safety awareness, did not use the machine in accordance with the manufacturer"s instructions, ignored the safeguards provided by protection covers, and risked his life by inserting a self-made steel pipe into seven steel rollers which were running faster than 20m/min.
Indirect cause:
Shanghai Liandong Technology Co. Ltd. had five shortcomings in its workplace safety management.
1)It had a long-term practice of ignoring workers’ unsafe behavior of using self-made machinery and tools;
2)Its safety instructions of drafter machine were not complete, did not specify requirements for safely carrying out extruding operations, and was not effective in guiding actual practices;
3)It did not require workers to use the machinery strictly in accordance with the manufacturer"s safety instructions;
4)It did not organize effective safety trainings among workers;
5)It did not spot and stop its employees’ unsafe behaviors.
Punishment for the accident:
Jinshan District Emergency Management Bureau imposed administrative penalties upon Shanghai Liandong Technology Co. Ltd.
Recommendations:
1. Shanghai Liandong Technology Co. Ltd. should treat this accident as a warning alarm, learn from this fatal accident, undertake deep-dive analyses to find the causes and design customized standard safety operation processes for its machinery. It should design a system to completely identify and remove safety hazards as well as implement it seriously.
2. Shanghai Liandong Technology Co. Ltd. should develop its internal work safety regulations, require its employees to strict comply with them and build a safe work environment. It should improve safety education training among its operators to enhance their safety awareness.
3. The local township government should take up its supervision responsibility to enhance its safety supervision and regulatory works for small and micro firms. The government should direct them to proactively identify and remove safety hazards and require them to shoulder their share of work safety responsibilities.
“2020·12·6” Struck by Object Accident at No.1818 Huiji Road
Summary of the accident:
At around 7:00, December 6, 2020, at Shanghai Yunjia Loading Service Co. Ltd. at No.1818, Huiji Road, Pudong New Area, an operator was hit by a boom fallen from a harbor crane under repairment.
Direct cause:
When the crane boom was being lowered, there was no safety round of hoist rope attached to the tip reel of the boom. The end of hoist rope alone could not hold the boom and slipped off the reel. The boom fell off and hit an operator who entered the reparation area against safety protocols.
Indirect cause:
Shanghai Yunjia Loading Service Co. Ltd. did not host effective safety trainings among its employees, which caused the now deceased worker ignoring safety requirements and entering the reparation area. The company did not properly manage the maintenance of the harbor crane and did not repair it according to standard procedures.
Punishment for the accident:
Pudong New Area Emergency Management Bureau imposed administrative penalties upon Shanghai Yunjia Loading Service Co. Ltd.
Recommendations:
1. Shanghai Yunjia Loading Service Co. Ltd. should learn from this fatal accident, amplify the learnings, report the accident details and investigation results to the whole company;
2. The company should effectively improve safety training programs among its employees, enhance their safety awareness and stop actions violating safety protocols to prevent future accidents.
3. The company should completely identify safety hazards at its job sites, enhance its safety risk management, continuously improve its safety incentive and penalty programs to strictly implement internal safety regulations and standard operation procedures.

➤When people come back to work after the Spring Festival holidays, accidents are more likely to happen because workers’ skills have become rusty, and they have lost focus on safety. The manufacturing machinery and systems might malfunction after long offline time. Extreme weather events are also likely to cause accidents. Businesses should enhance their trainings of operational skills and safety awareness to ensure all workers are qualified to take on their roles. Companies should spend more efforts in identifying and removing machinery hazards, as well as paying attention to stay safe during the hazard-removal works. Each business should take prevention measures and design customized preparedness plans for extreme weather events according to their individual industries.
